Adoption Forms
This section will show you the forms we use when we are considering an adoption. We call it an "adoption process" and the process usually takes several days to complete. Check our adoption contract to read the rules and regulations regarding our adoptions.
PLEASE NOTE: DO NOT FILL OUT ANY OF THE APPLICATION FORMS, AND MAIL TO US, WITHOUT CONTACTING US FIRST. WE WILL NOT REVIEW YOUR APPLICATION, OR CONSIDER YOU AS A PROSPECTIVE ADOPTIVE FAMILY, WITHOUT FIRST VISITING WITH YOU (via e-mail and phone) ABOUT YOUR PARTICULAR SITUATION. Also a submitted application does not guarantee adoption.
We use a series of forms to facilitate the adoption process. These forms are available in the Adobe PDF formats. PDF files (also used by the IRS for tax forms) require the Adobe Acrobat Reader, which can be downloaded free from Adobe at:

Preliminary Adoption Questionnaire (PDF: 53 KB/ 2 pages)
The preliminary form helps us to determine what type of animal you would like for your family companion. We also contact your veterinarian and references and visit with them in depth.Adoption Contract (PDF: 53 KB/2 pages)
This is the formal adoption contract that is completed when HTAS transfers ownership to the wonderful new adoptive family. We list all pertinent information at the bottom of the contract, i.e. shots, etc. so that the new family can tear off the bottom portion, take it to their vet and the vet clinic can start a new health record.Rental Agreement (PDF: 50 KB/1 page)
If you are currently renting a home or apartment, this form will need to be completed and signed by both you and your landlord as part of the application process.Release Form (PDF: 48 KB/1 page)
The release form needs to be signed as we use various pictures of our families for promotional purposes. See the Happy Tails section for some terrific pictures of our GREAT families and their GREAT new companions!
